Documentation 9-12

There are documents (important papers) that you must have for access to discounts, services and assistance. Discuss with your parents, counselor, or teacher, which of the documents you need and where they should be kept. Some of the documents will need to be kept in a safe place at home, while others will be carried with you.

You should only check one box for each item on the checklist. However, if you check the "Need" box, you will need to fill in the "Plan" box for that item.

Example:
