Family Activities to Support Transition
School is approximately one-fifth of a person’s life.  Students learn all of the time.  There are many ways that families can help students to learn the skills that they will need for the rest of their lives.  The ITP (Individualized Transition Plan) utilizes a process called Person Centered Planning, which focuses on 9 specific areas.  We have added 3 other sections, which are also related to transition.
