The links below list a range of activities done at school to assist your child to transition successfully to life after high school. The grades and years are not appropriate for all students, but a developmental guide to help your child with attaining independence after graduation from high school. Some students will not be able to reach these goals, and some will need considerable assistance to work on these goals. Please discuss this chart with your child’s IEP (Individualized Education Program) case manager, teachers, counselors and vocational rehabilitation counselor.
You can click on the link to the Sequential Transition Skills Blank Chart and print out a copy to bring to your child’s IEP/ITP meeting. You can complete this chart to match your child’s own Transition Plan as it develops yearly.
You can refer to the Sequential Transition Skills Sample Chart (color) to see the age and grade range when the various activities are available. Remember that students in the Special Education Program have the right to attend the San Diego City Schools until their twenty-second birthday.
