The work completed in the grades 9 12 portion of the Transition Website is the most crucial. It is the culmination of all the practice skills gained in the grades K 8 portion of the website. Although the Transition Website builds on skills and experience obtained in previous grades, students can begin using the materials at any stage. The 9th 12th grade section focuses on the actual documents and experiences students will need and use to get a job and transition from school to work.

Download and print PDF format and fill out hard (written) copy of forms
Download and print Microsoft Word format copies of forms and save forms in student folders on desktop or student disks
Students print rough draft of completed forms from Microsoft Word, they are edited (by teacher, peer or parent), revised in Microsoft Word and reprinted as a final copy.
Pick and choose appropriate materials to match student and class needs
Adapt forms to meet a variety of skill levels to fit student and class needs

The Transition Website has a consistent format to promote navigation and use. All sections begin with a grade level Table of Contents with clickable links to introductions, forms, examples or checklists. We have found that most students do not read the introductions. However, you may find them useful as a teaching resource. We have provided links to the student introductions at the end of each teacher introduction page.
